Pick the right form for each visit type — OPD, follow-up, psychiatry, discharge, and more.

A template is the shape of your note — which fields appear, what the AI listens for, and how the prescription is laid out. Your clinic has several templates installed. Pick the one that matches today's visit.


What templates are available

Common ones you'll see (your clinic may have more or fewer):

OPD Follow-upPatient returning for the same problem — shorter form, focused on response to treatment.
Psychiatry ConsultationFirst-time psychiatry visit. Adds AI insights — see Psychiatry insights.
Psychiatry Follow-upReturning psychiatry patient. Tracks medication adherence and symptom change.
Discharge SummaryEnd of an inpatient stay. Pulls in admission diagnosis, course, meds at discharge.
Emergency TriageCasualty/ER first contact. Fast vitals + complaint capture.

Don't see the template you need? Your clinic admin can add or customise templates from the Admin app. Ask them.


How to switch templates

Find the template name

Above the note area on the visit screen, the current template name shows in a row (for example, OPD General ▾). The small arrow ▾ means it's a dropdown.

Click and pick

Click the template name. A list drops down. Click the template you want.

The note area refreshes with the new fields. Anything you'd already typed in fields that exist in both templates is kept; fields unique to the old template are dropped.

Record as usual

That's it. Press the red mic and continue like a normal visit.

Switching templates mid-visit discards fields that don't exist in the new template. If you've already recorded and edited a lot, switch carefully or copy the important text into a notes app first.


When to switch — quick rules

  • Same patient, same complaint, second visit → use the matching follow-up template (e.g. OPD Follow-up, Psychiatry Follow-up). It's shorter and the AI pays more attention to "did the symptoms improve?".
  • Same patient, new complaint → switch back to the general template.
  • Discharge day → switch to Discharge Summary even if you've been using OPD General all week.
  • First-ever visit to your clinic → use the general template for that speciality.

Saving a template as your default

Your last-used template auto-loads next time. If you mostly use OPD General, you'll just see it open by default. No action needed.


Custom templates for your clinic

If you find yourself filling the same extra field every visit (e.g. a local registration number, a custom advice block), ask your clinic admin to add it to your template. They can do this from the Admin app without needing IT.
